As a new patient to our practice, to help facilitate in providing you with quality personal and dental care, we need to gain a thorough understanding of your medical and dental history. For this reason, we will request that you complete a Medical and Dental History form. For your convenience we have made this form available both online, alternatively however you can complete this at our practice in just a few minutes before your appointment.

Appointments

Your appointments are scheduled at specific times to best suit your needs. Should you be unable to attend an appointment, we request that at least 48 hours’ notice be given to avoid a cancellation fee. This will allow us to reschedule your appointment and accommodate other waiting guests. A deposit is required to reserve your selected appointment time.
